On this page you can upload a document such as a PDF or an image of a scanned document such as a form or birth certificate to the Fund Office. Unlike email, which is not secure, this upload page is secure, and your personal information is encrypted as it is sent to the Fund office. In order to identify you as the transmitter, you need to enter your Social Security number and date of birth to upload a document.

If the Fund Office needs to get a document to you, they will give you a retrieval "key" by phone or email. Using this page, you can then download the document securely by entering your Social Security number, date of birth, and the retrieval key.
