The due date for furnishing 2021 Form 1095-B to participants has been extended from January 31, 2022, to March 2, 2022.

All requests for a 1095-B will be mailed out. However, sending by secure email can be accommodated upon request.  You will receive your 1095-B within 30 days of your request.
