Death of Participant

Please notify the Fund office upon the death of a participant. The Fund office staff is ready to help as much as possible and to make sure that the survivors understand what benefits, if any, are payable by the Funds as a result of the participant’s passing.  The staff can also help survivors obtain any necessary forms to obtain the benefits and ensure that they understand how to submit the forms and any required documentation to the Fund office.
